Enter Registered Mobile Number

OTP Verification
You will receive an OTP (One-Time Password) on the mobile number you entered. Enter the OTP and the CAPTCHA code to log in.
Select State and District
After logging in, you will be directed to a new page. Select your state and district from the options provided.
Choose the Scheme
In the scheme section, click on "Pradhan Mantri Jan Arogya Yojana".
Verify Your Details
You will be asked to verify your details using one of the following options: Family ID, Aadhaar number, name, location, or PMJAY ID. Click the “Search” button.
View Your Ayushman Bharat Card
A new page will display all the Ayushman Bharat cards linked to your Aadhaar or Family ID. Select the card you want to download.
Download the Card
